Way too many details required. I would prefer a simple conflict of interest requirement to disclose current affiliations, e.g., current citizenship (or equivalent) and current positions in other regions and organizations. I could hesitantly vote for a conflict of interest requirement that also requires disclosure of past affiliations in other regions and orgs, though I'll note even this can be difficult for someone who's played for a few years. Anything more detailed than that, I simply cannot support because it will discourage people from running for office and possibly disqualify them if they make mistakes in their disclosure.
