It's not like the central government is the only government. You could nuke Washington D.C. but you still have the national government and its lower levels operating nationwide, and that's assuming every member of the executive was located right in the capital with absolutely no warning or preperation.

Possibly. Maybe he hid in a bunker and was warned in advance. Understand, In the U.S., we do have a "Safe Man", someone who essentially just sits in the presidential bunker during public events in case something like this happens.

By the way, 111 km is the distance at which anybody beyond would definitely suffer no damage from thermal radiation (heat).
If you wanted I could make an image for the approximate path of the fallout. I would only need the wind direction and average speed in the area, as well as the fission fraction of the nuke.

I'd apply Occam's razor. If the capital was nuked, but Flores survived, then Flores was not in the capital. Since he's the president, he was probably visiting some other city, doing presidential stuff.
