When and what is this survey??

Oh! Before I forget, We have stopped proliferation of chemical weapons (now we only have a stockpile)... and we have legislation for universal healthcare.

It's just a collection of multiple surveys considering many things like population, year of formation, and so forth. It has been going on for a couple of years now and it's regularly updating itself when needed.

Just have a question on the year of formation map. Is it based on the current incarnation of the country or when it first unified?

This is the survey that the map in question is based on. The map is meant to represent the year since which the nation has been continuously independent. So yeah, basically its most recent incarnation.
