The word casualty refers not only to deaths but also injuries, the nuke didn't "instantly wipe out" all of those people. Considering the nuke was detonated at ground level - and was somehow powerful enough to affect so many people - you should expect a whole lot of radioactive fallout. By what we know of TSP's general wind directions, most of the radioactive dust might be blown inland, which is bound to deal more damage.

Still, what exactly was the yield of this nuke, Henn?

The radioactive fallout would not cause immediate casualties though.

In that you are right. Though you might expect a significant increase in crop failure, cancer, etc in the long run. This nuke may have just as well crippled Sporaltryus.
